These are the consent forms for my two actors in my film the top form is for my main character which you don't really see, and the bottom one for the little boy crying over the graveyard. I didn't get them to fill in the whole form as then I would then need to blur it anyways, I decided not to blur the signature as they aren't really proper signatures.
